Most common D2631 code reviews : Pulpal Debridement, Primary or Permanent Tooth - Paid to the general dentist that will not be completing the endodontic treatment, Laboratory processing of microbial specimen to include culture and sensitivity studies, preparation and ransmission or written report or Posterior-anterior or lateral skull and facial bone survey film.

D2631 Procedures:

Non-autogenous connective tissue graft procedure (including recipient surgical site and donor material) - each additional contiguous tooth, implant or edentulous tooth position in same graft site. Local anesthesia is usually considered a component part of periodontal procedures, but dependent upon the plan will allow up to 50% of D2631 - allow up to a maximum of 3 teeth per quadrant

A detailed and extensive problem-focused evaluation entails extensive diagnostic and cognitive modalities ased on the findings of a comprehensive oral evaluation.D2631 integration of more extensive diagnostic modalities to develop a treatment lan for a specific problem is required The condition requiring this type of evaluation should be described and documented Examples f conditions requiring this type of evaluation may include dentofacial anomalies, complicated perio-prosthetic conditions, complex emporomandibular dysfunction, facial pain of unknown origin, severe systemic diseases requiring multi-disciplinary consultation.

Scaling and debridement in the presence of inflammation or mucositis of a single implant, including cleaning of the implant surfaces, without flap entry and closure. (When submitted with prophy, considered inclusive of prophy; no separate benefit for 6081 or when submitted alone or in multiples, allow to pay as prophy, but subject to prophy limitation.)

Re-cement or re-bond inlay, onlay, veneer or partial coverage restoration

Endodontic therapy - molar tooth (excluding final restoration). The fee for palliative treatment is Disallowed when done In Conjunction With root canal therapy by the same dentist/dental office on the same date of service. Palliative treatment is payable on a separate date of service for relief of pain. Incompletely filled root canals are not payable, and the fee for the endodontic therapy is Disallowed. Post removal is not included in this procedure.
